Strong Start Sports — Liability Waiver

Participant Liability Waiver & Release Agreement

By enrolling a child in Strong Start Sports programs, the parent or legal guardian acknowledges and agrees to the following terms and conditions.

Assumption of Risk

I understand that participation in physical fitness activities, sports training, outdoor movement programs, agility drills, strength exercises, and related activities involves inherent risks. These risks may include, but are not limited to, slips, falls, muscle soreness, strains, sprains, collisions, and other minor or serious injuries.

I voluntarily allow my child to participate in Strong Start Sports activities with full knowledge of the potential risks involved.

Medical Acknowledgment

I confirm that my child is physically able to participate in training sessions and does not have any medical condition that would prevent safe participation unless disclosed in writing.

I agree to inform Strong Start Sports of any medical concerns, allergies, behavioral considerations, physical limitations, or health conditions that may impact participation.

Release of Liability

I release and hold harmless Strong Start Sports, its owner, coaches, assistants, volunteers, staff members, contractors, and affiliated locations from any liability, claims, demands, damages, injuries, or expenses arising from participation in classes, training sessions, outdoor activities, or related programs.

This release applies to injuries, damages, or incidents occurring during participation except in cases of gross negligence or intentional misconduct.

Emergency Medical Authorization

In the event of an emergency, I authorize Strong Start Sports staff to seek medical treatment for my child if immediate care is necessary and I cannot be reached.

I understand that any medical expenses resulting from treatment are my responsibility.

Photo & Media Release

I understand that Strong Start Sports may occasionally take photos or videos during training sessions for promotional, educational, or marketing purposes.

Parents may choose whether to allow photo/media use during the registration process.

Behavior & Participation Expectations

I understand that Strong Start Sports maintains a positive, respectful, and structured environment for all participants.

Children are expected to follow coaching instructions, respect others, and participate safely.

Strong Start Sports reserves the right to remove a participant from a session if behavior creates safety concerns for themselves or others.

Weather & Cancellation Policy

Outdoor sessions may occasionally be cancelled due to weather conditions or unsafe environments.

If a session is cancelled by Strong Start Sports, a credit will be issued and no session value will be lost.

Agreement & Digital Signature

By checking the agreement box and submitting the booking form, I confirm that:

• I am the parent or legal guardian of the participating child
• I have read and understood this Liability Waiver
• I voluntarily agree to all terms outlined above
• I accept responsibility for my child’s participation
• The information provided during registration is accurate
